As a participant in a position within the league, you will need to supply the following information:

Social Security Number

Drivers or Identification Card Number

Banking Account Information (routing and account number)
for direct deposit​

EMPLOYMENT INFORMATION
You will be asked to complete a W-9 form (fillable PDF), save it to your desktop, and then upload it during the registration process. Also, at the end of the registration process, there will be a link for you to complete a direct deposit form. You will need your banking account information (found on a blank check) readily available. These forms will then be sent directly to the TKCSA business office.
REFEREE & SCOREKEEPER TRAINING
Mandatory training for Referees and Scorekeepers will be provided Registrants will be asked to indicate which ONE of two available sessions they plan to attend.
